Job Summary
NuChoice Health Partners, a home care agency serving the Northwest of Pennsylvania, is seeking a dedicated Personal Care Assistant to join our team. As a Caregiver, you will provide essential support to patients in their own homes, promoting independence, comfort, and overall well-being.
Responsibilities
- Assist patients with daily living activities such as bathing, grooming, and dressing
- Prepare and cook meals, promoting proper nutrition and hydration
- Provide medication reminders and monitor medication adherence
- Offer companionship and emotional support to patients, enhancing their mental health and social well-being
- Help patients with mobility exercises and maintain a safe and clean environment
- Collaborate with healthcare professionals to ensure the well-being of patients
- Document tasks completed and time in and out, maintaining accurate records

Work Environment
