About the Role
We are seeking a skilled and compassionate Skilled Homecare Caregiver (LPN) to join our dedicated team of healthcare professionals. As a member of our care team, you will provide high-quality, personalized care to patients in their homes, promoting independence, dignity, and well-being. This is a PART_TIME_AND_FULL_TIME opportunity that offers flexible scheduling and a rewarding career.
Responsibilities
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
- Conduct patient assessments, develop care plans, and collaborate with healthcare teams to ensure comprehensive care
- Administer medications, perform treatments, and provide supportive care as needed
- Maintain accurate and detailed records of patient care and communication with healthcare teams
- Provide emotional support and education to patients and their families, promoting independence and coping skills
Requirements
- LPN license in the State of Pennsylvania
- 2 years of experience in home care or a related field
- Ability to work independently and as part of a team
- Excellent communication, interpersonal, and problem-solving skills
- Strong attention to detail and organizational skills
- Ability to lift, transfer, and move patients with the use of equipment as needed
