Job Summary
NuChoice Health Partners, a leading home care agency serving Mercer, Lawrence, Butler, Venango, and Crawford Counties in Pennsylvania, is seeking a dedicated Personal Care Assistant (Caregiver) to join our team. As a Caregiver, you will provide essential support to patients in their homes, ensuring they receive the highest level of care and comfort.
Responsibilities
- Assist patients with daily living activities such as bathing, grooming, and dressing
- Prepare and cook meals, as needed
- Provide medication reminders and adhere to medication schedules
- Offer companionship and emotional support to patients
- Assist with mobility exercises and maintain a safe and clean environment
- Collaborate with healthcare professionals to ensure the well-being of patients
- Document tasks completed and time in and out
