Job Description

The Case Manager/Utilization Review professional will play a crucial role in supporting the Case Management Department at Chestnut Hill Hospital. This position focuses on utilization review and coordinated discharge planning for patients across all hospital areas, including those in behavioral health and psychiatric care. The Case Management team is committed to ensuring safe and appropriate discharge plans based on individual patient needs.

Responsibilities

• Provide discharge planning support for patients throughout all areas of the hospital.
• Perform utilization review and case management functions.
• Coordinate discharge planning for inpatient populations, including psychiatric and behavioral health patients.
• Collaborate with hospital departments and care teams to facilitate patient transitions and discharge needs.
• Assist in identifying and coordinating resources necessary for effective discharge plans.
• Support continuity of care throughout the discharge planning process.
• Maintain appropriate documentation within the EPIC electronic health record system.

Required Experience

Experience in case management and utilization review within a hospital setting is preferred.

Certifications/Licensure

Valid nursing licensure is required.
