Job Summary

We are seeking a dedicated and detail-oriented Admissions Coordinator to join our team at Wyomissing Health and Rehabilitation Center, a 103-bed facility located in Reading, Pennsylvania. As a key member of our Admissions department, you will play a critical role in ensuring a smooth transition for new patients and their families, while providing exceptional customer service and support.

Admissions Coordinator Responsibilities

• Coordinate the intake process for new patients, including scheduling admissions, conducting tours, and gathering necessary documentation
• Develop and maintain relationships with referral sources, hospitals, and communities to generate admissions and increase visibility for the center
• Manage and maintain accurate records and reports, including patient census and admissions data
• Collaborate with other departments to ensure seamless transitions for patients and families
• Provide information and support to patients, families, and referral sources to address questions and concerns
• Participate in quality improvement initiatives to optimize admissions processes and patient satisfaction
• Maintain confidentiality and adhere to HIPAA regulations

Requirements

• High school diploma or equivalent required; Associate's or Bachelor's degree in a related field preferred
• Prior experience in healthcare or related field; training in medical terminology and admissions processes a plus
• Excellent communication and interpersonal skills
• Ability to work efficiently in a fast-paced environment with multiple priorities
• Proficiency in electronic medical records systems and Microsoft Office
