Job Summary
Case Management professional responsible for supporting utilization review activities for inpatient, observation, and swing-bed patients in an acute care setting. The role manages approximately 8–10 patients per day and collaborates on case management activities involving conditions such as chronic obstructive pulmonary disease, congestive heart failure, and debility. The position follows a weekday schedule with no weekends, holidays, or on-call responsibilities.
Job Details
- Required Qualifications: Minimum of 2 full years of case management experience; Basic Life Support (BLS) certification from the American Heart Association (AHA); insurance portal experience; active professional licensure.
- Preferred Qualifications: Certified Case Manager (CCM) certification preferred.
- Schedule: Day shift, Monday through Friday, 7:00 AM–3:30 PM; 8 hours per shift and 40 hours per week.
- Additional Information: Non-clinical case management role focused on utilization review for notices of admission. Responsibilities support inpatient, observation, and swing-bed populations, including patients with chronic obstructive pulmonary disease, congestive heart failure, and debility. The expected caseload is approximately 8–10 patients per day. Discharge planning support includes pharmacy coordination. Department-specific orientation consists of approximately 2–3 shifts.
