Responsibilities
- Provide high-quality patient care and management in a fast-paced rehabilitation setting
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
- Demonstrate strong communication and interpersonal skills to effectively interact with residents, families, and colleagues
- Participate in quality improvement initiatives to drive continuous improvement and excellence in patient care
