Job Description

The Clinic Registered Nurse will provide evidence-based nursing services to participants enrolled in the PACE program. Responsibilities include supporting participants and their caregivers, aiding participants in maintaining or improving health through clinic services such as teaching, counseling, and skilled interventions. The role requires superior physical assessment techniques, effective nursing care plan development, and professional nursing care.

Responsibilities

• Provide skilled nursing interventions to participants in the clinic setting.
• Develop and implement individualized nursing care plans in collaboration with the healthcare team.
• Conduct thorough physical assessments and document findings accurately.
• Educate and counsel participants and family members on health maintenance and improvement strategies.
• Coordinate and manage scheduling, communication, and documentation of all care provided to ensure compliance with regulatory requirements.

Required Experience

• Registered Nurse with current Colorado licensure.
• Graduated from an accredited school of nursing; BSN preferred.
• Minimum of 2 years experience as an RN, with clinic experience preferred.

Certifications/Licensure

• Current Colorado RN licensure required, maintained throughout employment.
