Schedule/Shift

Day Shift: 8:00 AM - 4:30 PM. Required to work the day after Thanksgiving, Christmas Eve, and New Year’s Eve.

Job Description

The Case Manager will embody the Select Medical Way and Core Values, ensuring high-quality patient care and safe working environments for employees. The role focuses on patient safety, requiring interaction with patients and families to address barriers to treatment and facilitating discharge planning in collaboration with the interdisciplinary team.

Responsibilities

• Identify patient problems affecting their treatment and assist in utilizing hospital resources.
• Collaborate with the interdisciplinary team, including physicians, to manage clinical plans of care.
• Facilitate family involvement in decision-making for individual care plans and discharge safety.
• Notify management of major changes in patient conditions and emergencies.
• Maintain accurate and timely clinical documentation per facility policies and payor guidelines.
• Manage and direct the clinical plan to ensure timely patient discharge.
• Engage with community resources to assist patients and families in accessing needed services.

Required Experience

• Current licensure in a clinical discipline (RN, RT, SW preferred).
• Working knowledge of healthcare insurance and government reimbursement.
• Minimum of 2 years experience in Case Management and Discharge Planning in an Acute Care setting.

Certifications/Licensure

Active licensure in nursing or relevant clinical discipline per state guidelines is required.
