Job Description

The Case Management unit primarily focuses on discharge planning and managing patient cases to ensure a smooth transition from hospital to home or other care facilities. The unit consists of 20 beds, with a dedicated team of Case Managers and Social Workers to manage the patient load. The equipment utilized includes Mobiles and electronic medical records via Meditech and CarePort.

Responsibilities

• Conduct assessments and create discharge plans for patients.
• Specialize in adult discharge planning, palliative care, and goals of care conversations.
• Collaborate with care teams to facilitate communication and patient needs.

Required Experience

• Minimum 1-year current Case Management RN experience.

Certifications/Licensure

• Active RN license in the state of Colorado.
