Job Description

The Registered Nurse in Case Management will play a critical role in providing support to patients by coordinating care across various services. This role requires collaboration with various healthcare professionals to ensure seamless transitions and continuity of care for patients.

Responsibilities

• Work collaboratively with RN and MD.
• Maintain excellent documentation skills.
• Effective communication with patients, families, and the healthcare team.
• Manage multiple tasks and shift attention as needed.
• Build trust with patients and their families to enhance care relationships.

Required Experience

Previous experience in case management or a related nursing field is preferred.

Certifications/Licensure

Current RN licensure in the state of North Carolina is required.
