Schedule/Shift

Scheduled shifts are 4x10 hours, with days likely starting at 07:00 AM. Weekend coverage is required every other weekend.

Job Description

The Registered Nurse, Case Manager will focus on discharge planning and care coordination for patients in an acute hospital setting. The position requires working with a diverse patient population, including neonates, adolescents, adults, geriatrics, and pediatrics. Familiarity with Epic as an EMR is essential, and candidates will collaborate with interdisciplinary support teams including physical therapy, respiratory services, social services, and pharmacy.

Responsibilities

• Manage patient assessments and develop plans of care.
• Conduct continued stay reviews and pre-certification reviews for medical necessity.
• Coordinate with various hospital teams to ensure seamless patient transitions.
• Adhere to HIPAA guidelines and ensure compliance with The Joint Commission standards.
• Utilize InterQual criteria and Milliman guidelines in case management processes.

Required Experience

• A minimum of 1 year of acute care experience is required.
• Epic experience is mandatory.

Certifications/Licensure

• Current Alaska RN license is required.
• BLS certification is preferred.
• ACM certification is preferred.
