This is a 13-week contract position.
Schedule/Shift
The shift will be either a 4x10 or 5x8 schedule during the day. Shift times may vary but will be one of the following: 07:00-15:30, 07:30-16:00, 08:00-16:30, 08:30-17:00, 07:00-17:30, or 07:30-18:00. Weekend coverage is required, typically every other weekend or 4 weekend days in a 4-week schedule.
Job Description
As a Case Manager RN at our acute hospital, you will be responsible for discharge planning and care coordination for patients, utilizing your Epic experience with acute care settings. The hospital is recognized as a Magnet Facility and a Level II Trauma Center with 407 staffed beds, and you will work as part of an interdisciplinary team to meet patient needs.
Responsibilities
- Coordinate discharge planning for patients to ensure smooth transitions of care.
- Utilize Epic for documentation and care management.
- Collaborate with interdisciplinary teams including social services, pharmacy, and respiratory therapy.
- Review and assess medical necessity based on evidence-based guidelines.
- Participate in continued stay reviews and pre-certification processes.
Required Experience
- Minimum of 1 year of acute care experience.
- Epic experience is a must.
Certifications/Licensure
- Current Alaska RN License is required for an auto offer.
- BLS certification is preferred; ACM certification is also preferred.
