Schedule: Monday to Friday, 5 × 8-hour shifts, 7:00 AM - 3:00 PM.

Job Description

The Licensed Practical Nurse will provide essential care and support to patients in home health settings within Rankin County. This position may require traveling to various locations within a 60-mile radius of the facility.

Responsibilities

• Conduct routine patient visits in accordance with care plans.
• Document patient progress and care using HomeCare HomeBase (HCHB).
• Collaborate with healthcare teams to ensure quality patient care.
• Manage patient caseload to meet the requirement of 32 points per week.

Required Experience

• Recent home health experience is mandatory.
• HomeCare HomeBase experience is required; candidates may be considered at a reduced rate for the first two weeks if willing to train without prior HCHB experience.

Certifications/Licensure

• Valid LPN license in the state of Mississippi.
