Alpine Care of St. Charles is seeking a dedicated and experienced MDS Coordinator RN/LPN to join our team in St. Charles, IL. Responsibilities

• Conduct MDS assessments and reviews to ensure accurate and comprehensive data collection and reporting
• Coordinate with interdisciplinary team members to gather information and complete MDS assessments
• Ensure that all MDS assessments are completed in a timely and thorough manner, meeting all regulatory and accreditation requirements
• Develop and implement effective strategies to improve MDS accuracy and completeness
• Provide education and training to staff on MDS processes and procedures
• Maintain accurate and up-to-date records and reports
• Collaborate with other departments to ensure seamless communication and coordination

Requirements

• Current RN or LPN licensure in the state of Illinois
• Minimum of 1 year of experience in a healthcare setting, preferably in long-term care or rehabilitation
• Strong understanding of MDS assessment processes and regulations
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment with multiple priorities
• Strong attention to detail and organizational skills
