Social Worker
The Wesley Community is seeking a compassionate and experienced Social Worker to join our team on a full-time basis. As a Social Worker, you will be responsible for providing social work services to residents in our long-term care units. About the Wesley Community
Responsibilities
- Provide social work services to residents in our long-term care units, including assessing social and emotional needs, developing and implementing care plans, and providing counseling and support.
- Maintain accurate and timely documentation of interactions with residents and families.
- Collaborate with other healthcare professionals to ensure coordinated care and services.
- Provide education and support to residents and families on topics such as long-term care options, insurance, and benefits.
- Develop and maintain relationships with community agencies and organizations to ensure access to resources and services for residents.
Requirements
- Bachelor's degree in social work or human services field.
- Geriatric experience preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Must be able to work day shifts.
