MDS Coordinator – RN
The Mariemont Care Center is looking for an experienced MDS Coordinator to join our nursing team.
Responsibilities
- Coordinate and complete MDS assessments accurately and on time
- Manage the Resident Assessment Instrument (RAI) process
- Develop and update individualized care plans
- Coordinate with nursing, therapy, dietary, social services, and other departments
- Participate in interdisciplinary care plan meetings
- Review clinical documentation for accuracy and compliance
- Monitor MDS schedules and required deadlines
- Ensure documentation supports resident care and reimbursement
- Maintain compliance with CMS, Medicare, Medicaid, and state regulations
Qualifications
Current, active RN license in Ohio
- Previous MDS experience required
- Long-term care/skilled nursing experience preferred
- Knowledge of MDS 3.0, RAI, and care planning
- Strong attention to detail and organization
- Ability to work independently and meet deadlines
- Strong communication and teamwork skills
