Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our team at The Lodge at Heritage Grove Estates, a skilled nursing facility (SNF) in Topeka, KS. As a Physical Therapist with our organization, you will have the opportunity to make a meaningful difference in the lives of our residents by providing high-quality, patient-centered care. Responsibilities
- Assess, plan, and implement individualized physical therapy programs to meet the unique needs of our residents
- Conduct thorough evaluations and develop comprehensive treatment plans to improve mobility, strength, and overall function
- Implement and document patient treatments, ensuring accurate and timely documentation
- Collaborate with other healthcare professionals to develop and implement interdisciplinary care plans
- Monitor and report patient progress to family members and healthcare team members
- Maintain accurate and up-to-date records and reports as required
Requirements
- Bachelor's or Master's degree in Physical Therapy from an accredited institution
- Valid Kansas license as a physical therapist
- Minimum of 0 years of experience in a skilled nursing facility (SNF) or related healthcare setting
- Strong communication and interpersonal skills
- Ability to lift and transfer patients as needed
