The Gardens at Country, a leading healthcare provider in Swansea, Massachusetts, is seeking a dedicated and compassionate Evening Shift LPN to join our team!
About the Role
We are committed to delivering exceptional care to our residents and are looking for a skilled and experienced Licensed Practical Nurse (LPN) to work our evening shift. As an Evening Shift LPN at The Gardens at Country, you will have the opportunity to make a meaningful difference in the lives of our residents while working in a collaborative and supportive environment. Responsibilities
- Provide direct patient care to our residents, including medication administration, wound care, and vital sign monitoring
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with other members of the healthcare team to ensure seamless care transitions and effective communication
- Demonstrate a commitment to excellence in care delivery, adhering to established standards and protocols
- Participate in quality improvement initiatives to continuously improve resident outcomes and satisfaction
Requirements
- Current LPN license in the state of Massachusetts
- Completion of an accredited LPN program
- Strong clinical and communication skills
- Ability to work effectively in a fast-paced environment
- Compassionate and empathetic approach to care
