Home Care Companion

This is a part-time position in our care coordination department. Typical schedule is within the hours of Monday - Friday, 8:00AM - 4:30PM.

Job Summary: Under the direction and supervision of the In-Home Services CNA Supervisor, the Healthcare Companion and Delivery Assistants responsible for providing participants with 1-1 assistance with activities of daily living, transfers, ambulation, and ensuring safety during external medical appointments.

Duties and Responsibilities:

Consults each day with the In-Home Services CNA Supervisor to be updated on St. Paul’s PACE participant’s status, changes in care plan and to confirm scheduled visits.

Independently follow the assigned schedule, ensuring timely arrival at all scheduled visits.

Report cancellations, unusual incidents, and any other pertinent information to the appropriate staff members in real time.

Document services provided in participant’s electronic medical record.

Assist with coordination and scheduling of medical companion services as needed.

Provide any needed assistance in the home to prepare for medical appointments i.e. bathing, grooming, meal prep, etc.

Personal ADL assistance (toileting, hygiene, dressing) as needed.

Assists participants with transfers and ambulation

Provide supervision throughout medical appointments as required to ensure participant safety

Other duties as assigned

Qualifications and Requirements:

Education: High School diploma or equivalent required. If CNA, completion of formal CNA training program. Successful completion of First Aid and CPR training in person with the American Heart Association. For CNAs, current California CNA certification. Valid driver’s credentials and reliable transportation is required

Experience: Minimum of one year’s experience working with the frail or elderly
