Physical Therapist Assistant (PTA)

Saunders Nursing and Rehabilitation Center is seeking a full-time Licensed Physical Therapist Assistant (PTA) to join their team!

The primary responsibility of the Physical Therapy Assistant (PTA) is to provide direct physical therapy interventions as planned, delegated, and supervised by the Physical Therapist. The person in this role will also record treatments given and note progress in medical records. In this role, you are entrusted to ensure that the highest degree of quality care can be always provided to our residents.

Responsibilities

• Assist the Physical Therapist in carrying out treatment plans and gathering objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
• Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
• Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
• Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service (PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
• Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conduct remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
• Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.

Qualifications

• Education: Graduation from an accredited Physical Therapist Assistant program.
• Licensure: Active state licensure as a Physical Therapist Assistant.
• Experience: Prior experience in a subacute rehabilitation setting is preferred but not required.
• Skills: Strong communication abilities and a collaborative team spirit.
