Job Summary
Ridgewood Manor, a Medicare & Medicaid certified skilled nursing center, is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in Maumee, Ohio. As a valued member of our healthcare family, you will play a pivotal role in providing exceptional care to our patients, with a focus on rehabilitation therapy and long-term skilled nursing care. Responsibilities
- Provide high-quality, patient-centered care to residents, including administering medications, performing treatments, and monitoring vital signs
- Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team
- Coordinate and communicate with family members, physicians, and other healthcare providers to ensure seamless care transitions
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
- Maintain accurate and up-to-date patient records and documentation
