Job Summary

We are seeking a highly skilled and compassionate Social Services Director to join our team. As a Social Services Director, you will play a vital role in ensuring the social, emotional, and spiritual well-being of our residents. You will be responsible for developing and implementing comprehensive care plans, conducting assessments, and providing support services to our residents and their families.

Responsibilities

• Develop and implement individualized care plans, considering the physical, emotional, and social needs of each resident
• Conduct thorough assessments of residents' needs, including social, emotional, and spiritual needs
• Provide guidance and support to residents, families, and staff to address concerns and issues
• Collaborate with other healthcare professionals to ensure a holistic approach to care
• Develop and maintain relationships with community resources and providers to access additional services and support
• Maintain accurate records and reports, including documentation of assessments, care plans, and interventions
• Participate in quality improvement initiatives to enhance the overall quality of care

Requirements

Masters degree in Social Work, Psychology, or a related field

• Minimum 3 years of experience in social services or a related field
• Strong knowledge of social services principles, practices, and regulations
• Excellent communication and interpersonal skills
• Ability to work effectively with diverse populations and maintain confidentiality
• Strong organizational and time management skills
