Job Title: Minimum Data Set Nurse

Job Summary
Parkview Care Center, a leading provider in post-acute care and senior healthcare services, is seeking a highly skilled and compassionate Minimum Data Set (MDS) Nurse to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring our residents receive the highest quality care and services. Responsibilities
- Conduct thorough patient assessments, including minimum data set (MDS) assessments, to identify patient care needs and develop individualized care plans
- Collaborate with the interdisciplinary team to implement care plans, providing education and guidance to patients, families, and caregivers
- Monitor and report changes in patient condition, and make recommendations for revisions to care plans as needed
- Participate in quality improvement initiatives, including data collection and analysis, to ensure optimal patient outcomes
- Maintain accurate and timely documentation of patient assessments, care plans, and progress
