Orchard Manor Skilled Nursing & Rehabilitation is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our exceptional nursing team. Available Positions
Full-Time & Part-Time
- Rehab Unit: Full-Time Opening
- Day Shift: 6:00 AM – 2:00 PM
- Evening Shift: 2:00 PM – 10:00 PM
- Night Shift: 10:00 PM – 6:00 AM
- Baylor Weekend Shift: 6:00AM- 10:00PM

LPN Responsibilities
- Provide high-quality nursing care in accordance with physician orders and individualized care plans
- Administer medications, treatments, and nursing procedures safely and accurately
- Monitor residents' conditions and promptly report changes to the RN, physician, or appropriate healthcare provider
- Supervise and support Certified Nursing Assistants (CNAs)
- Maintain accurate and timely electronic medical records (EMR)
- Communicate effectively with residents, families, physicians, and interdisciplinary team members
- Follow all infection prevention, safety, and regulatory guidelines to ensure quality resident care

Qualifications
- Current, active Licensed Practical Nurse (LPN) license
- Long-term care, skilled nursing, or rehabilitation experience preferred
- Strong clinical assessment and communication skills
- Compassionate, dependable, and resident-focused
- Ability to work effectively as part of a multidisciplinary team
