We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Mosholu Parkway Nursing & Rehabilitation Center in The Bronx, New York. As a valued member of our staff, you will have the opportunity to work with a diverse group of patients, providing high-quality care and making a meaningful difference in their lives.
Responsibilities
- Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Conduct vital signs, monitor patient conditions, and report changes to nursing staff
- Participate in providing education and support to patients and families
- Maintain accurate and up-to-date patient records
- Assist with scheduling and documenting doctor's orders and treatments
Requirements
- Current LPN license in the State of New York
