About the Role

• Provide psychosocial assessments, interventions, and counseling to residents and families to address their emotional, social, and psychological needs
• Develop and implement individualized care plans, while collaborating with interdisciplinary teams to ensure comprehensive care
• Foster a positive and supportive environment, promoting a sense of community and dignity among residents
• Document resident progress, maintain accurate records, and participate in care coordination and discharge planning
• Develop and maintain relationships with families, staff, and community resources to ensure ongoing support and care

Responsibilities

• Conduct initial and ongoing assessments of residents' needs, interests, and abilities
• Develop and implement individualized plans of care, including interventions and activities to promote resident well-being and satisfaction
• Provide counseling and support to residents and families, addressing issues such as depression, anxiety, grief, and trauma
• Collaborate with interdisciplinary teams, including nursing, therapy, and dietary staff, to ensure comprehensive care and coordination
• Participate in care conferences, staff meetings, and other committees as required
• Maintain accurate and timely records, including assessments, progress notes, and discharge summaries
