Mill Creek Center for Nursing & Rehabilitation is seeking a licensed Physical Therapist Assistant (PTA) to join our supportive and collaborative rehabilitation team. In this role, you’ll work directly with residents to help them improve strength, mobility, independence, and overall quality of life.

Position Details

• Status: Full-Time
• Schedule: Day Shift
• Setting: Skilled Nursing / Long-Term Care

What You’ll Do

• Provide hands-on therapy services under the direction and supervision of a Physical Therapist.
• Follow individualized treatment plans designed to support each resident’s goals.
• Provide individual and group therapy as appropriate.
• Monitor resident progress and communicate changes to the Physical Therapist and interdisciplinary team.
• Complete accurate and timely daily treatment notes and required documentation.
• Assist with discharge planning, resident education, and transitions of care.
• Collaborate with nursing staff, therapy team members, physicians, residents, and families.
• Participate in care conferences and other facility initiatives.
• Promote resident safety, dignity, mobility, and independence.

What We’re Looking For

• Graduate of an accredited Physical Therapist Assistant program.
• Current North Carolina PTA license.
• Previous skilled nursing or long-term care experience preferred, but not required.
• Strong communication, organization, and time-management skills.
• Ability to work effectively as part of a multidisciplinary team.
• Compassionate, dependable, and resident-focused approach.
• Ability to safely assist residents with mobility and therapeutic activities.
