Job Summary
We are seeking a compassionate and dedicated Dayshift LPN to join our team at Meadowbrook Care Center. As a Dayshift LPN, you will play a vital role in providing high-quality care to our residents, while working collaboratively with our interdisciplinary team to ensure exceptional outcomes.
Responsibilities
- Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians
- Conduct vital signs, monitor resident conditions, and report changes to the healthcare team
- Develop and implement individualized care plans, in conjunction with the interdisciplinary team
- Collaborate with rehabilitation staff to develop and implement treatment plans
- Participate in quality improvement initiatives to ensure optimal resident outcomes
- Maintain accurate and timely documentation of resident care and services
- Assist with admissions, transfers, and discharges as needed
- Perform other duties as assigned by the RN or Nurse Manager
Requirements
- Current LPN licensure in the state of Ohio
- 0 years of experience (new graduates welcome to apply!)
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong attention to detail and ability to prioritize tasks
