Do you enjoy building relationships with seniors, supporting caregivers, and helping people receive dependable care? Interim HealthCare of St. Louis is seeking a compassionate, organized, and hands-on Community Home Care Coordinator to join our team.

As our primary contact at an Independent Living community, you’ll help residents get started with services, support current clients and caregivers, and build connections with residents, families, and community staff. You’ll also provide field support and direct care throughout our home care service area when needed.

This salaried position requires flexibility to meet the needs of the community, clients, caregivers, and daily workflow, including occasional evening or weekend support.

What You’ll Do

  • Welcome new clients: Complete home visits and nonclinical care assessments, explain services and rates, complete enrollment paperwork, and coordinate starts of care.
  • Support caregivers: Provide check-ins, onsite observation, coaching, and timely communication about attendance, performance, and safety concerns.
  • Complete caregiver introductions: Meet caregivers at client homes, review care plans and preferences, and help ensure a comfortable first shift.
  • Support training: Complete caregiver skills assessments and case-specific training for clients with higher care needs, documenting skills and addressing gaps before independent assignments.
  • Help maintain coverage: Partner with recruitment on staffing needs and fill open care shifts at the community and throughout our service area as needed.
  • Build community relationships: Connect with residents, families, and community staff through events, introductions, outreach, and follow-up.
  • Highlight positive experiences: Support approved social media activities and invite voluntary, honest client and family reviews.
  • Keep the team informed: Maintain accurate AxisCare documentation, attend weekly office team meetings and a weekly supervisor meeting, and follow through on care, staffing, and outreach priorities.

What We’re Looking For

  • Experience in home care, senior living, caregiving, care coordination, or a related service role.
  • Experience guiding caregivers and conducting client visits preferred.
  • Strong communication, organization, problem-solving, and follow-through skills.
  • A warm, professional approach to discussing services with prospective clients and families.
  • Ability to adapt to changing priorities and take ownership of client and caregiver needs.
  • Ability to provide assigned direct care within your training and client care plans.
  • Reliable transportation and ability to travel throughout assigned home care territories.
  • Ability to meet applicable hiring, driving, and training requirements.
  • Commitment to client confidentiality, safety, and dependable service.
