About the job
Job Summary
Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our pediatric home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.
Job Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
- Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
- Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
- Collaborate with the clinical team to ensure coordinated and effective treatment
- Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
Job Qualifications
- Active New York State Physical Therapy License (PT or PTA) - required
- Basic computer proficiency for electronic documentation
- Strong clinical assessment and interpersonal communication skills
- Ability to work independently in a home health environment
- Valid NY driver's license and reliable transportation for home visits
- Pediatric experience preferred
- Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
Job Details

- Salary: $45–$70 per hour
- Schedule: Flexible - choose your own hours; day and evening shifts available, Monday–Friday, with weekend availability as needed
