Physical Therapist (PT) or Physical Therapist Assistant (PTA) – Home Health
Job Summary
Homeside Rehab is seeking a compassionate and skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our home health team in Oswego County, NY 13135.
As a valued member of our team, you will provide individualized, evidence-based therapy to patients in their homes while enjoying schedule flexibility and support from our clinical and administrative team.
Job Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
- Provide evidence-based physical therapy interventions to improve mobility, strength, balance, functional independence, and overall quality of life
- Maintain accurate and timely documentation of all patient care in accordance with regulatory requirements
- Collaborate with the clinical team to ensure coordinated and effective treatment
- Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
Job Qualifications
- Active New York State Physical Therapy license appropriate to discipline
- Basic computer proficiency for electronic documentation
- Strong clinical assessment and interpersonal communication skills
- Ability to work independently in a home health environment
- Valid driver's license and reliable transportation for home visits
- Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
