Community Care Givers is seeking a compassionate, motivated, and dependable Physical Therapist (PT) to join our growing home health care team! If you enjoy providing personalized care, helping patients regain independence, and working in a flexible environment, we would love to hear from you!
Responsibilities
- Provide physical therapy services to patients in their homes.
- Assess patient needs and develop individualized treatment plans.
- Help patients improve mobility, strength, balance, and overall independence.
- Monitor patient progress and adjust treatment plans as needed.
- Educate patients and families on exercises, safety, and injury prevention.
- Collaborate with nurses, physicians, and other healthcare professionals.
- Complete accurate and timely documentation.
Qualifications
- Current Physical Therapist (PT) license in the State of Michigan.
- Graduate of an accredited physical therapy program.
- Strong communication, organizational, and time-management skills.
- Ability to work independently and manage patient visits.
- Valid driver’s license and reliable transportation.
- Previous home health experience preferred but not required.
