About the job
Position Summary The PRN Physical Therapist evaluates and treats short-term rehab and long-term residents to restore mobility, strength, and function, covering caseload needs as scheduled.
Key Responsibilities
- Perform PT evaluations and re-evaluations; establish goals and plans of care
- Deliver skilled treatment: gait and balance training, therapeutic exercise, transfers, and modalities
- Supervise PTAs and rehab aides per Pennsylvania practice regulations
- Document evaluations, daily notes, progress reports, and discharge summaries per Medicare and payer requirements
- Participate in care conferences and coordinate with nursing on functional status and safety
- Recommend DME and provide family and caregiver training for discharge
Qualifications
- Current Pennsylvania Physical Therapist license
- Current BLS/CPR certification
- SNF or sub-acute experience preferred; knowledge of Medicare Part A/B and PDPM documentation
