Grafton Oaks is under new management and we’re growing fast! We’re looking for caring and reliable Licensed Practical Nurses (LPNs) to join our nursing team. If you're passionate about long-term care and want to work in a stable, supportive facility - we want to meet you.

Responsibilities
- Deliver safe, effective nursing care in accordance with resident care plans
- Administer medications and treatments per physician orders
- Monitor residents for changes in condition and report as needed
- Maintain accurate documentation and ensure compliance
- Build relationships with residents and families
Requirements
- Active LPN license in Ohio
- Long-term care or skilled nursing experience preferred (not required)
- Great communication, reliability, and a positive attitude
- Team player mentality
