About the job
Delmar is seeking a compassionate and dedicated Social Worker to join our nursing home team. The Social Worker will assist residents and their families with emotional, social, and discharge planning needs while promoting the highest quality of life and resident well-being.

Qualifications
-Bachelor's degree in social work required; master's degree preferred.
-Previous experience in a nursing home or healthcare setting preferred.
-Strong communication, organizational, and interpersonal skills.
Responsibilities
-Conduct resident assessments and develop individualized care plans.
-Provide emotional support and counseling to residents and families.
-Coordinate discharge planning and community resources.
-Assist with resident advocacy and rights.
-Participate in care plan meetings and interdisciplinary team discussions.
-Maintain accurate documentation and ensure regulatory compliance.
