Crystal Lake Healthcare and Rehabilitation, a 235-bed skilled nursing and rehabilitation facility, is seeking a dedicated, compassionate, and organized Social Worker (SW/CSW) to join our team in Berkeley Township, New Jersey. Our facility provides long-term skilled nursing care and short-term post-acute rehabilitation services.

Position Details

• Schedule: Monday through Friday
• Hours: 8:30 AM – 4:30 PM
• Employment Type: Full-Time
• Setting: Skilled Nursing Facility (SNF) / Long-Term Care / Rehabilitation

Key Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized resident care plans
• Coordinate discharge planning to ensure safe and successful transitions home or to other care settings
• Provide emotional support, counseling, and guidance to residents and their families
• Collaborate with nursing, rehabilitation, administration, and other interdisciplinary team members
• Assist residents and families with community resources, support services, and care-related decisions
• Participate in care plan meetings and maintain effective communication with residents and families
• Address resident concerns and advocate for their rights, dignity, and overall well-being
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements
• Support admissions, care transitions, and ongoing resident adjustment to the facility

Qualifications

• Bachelor's degree in Social Work (BSW) or Master's degree in Social Work (MSW) from an accredited institution
• New Jersey Certified Social Worker (CSW) or applicable social work license preferred
• Minimum of 2 years of healthcare or social work experience preferred
• Experience in a skilled nursing facility, long-term care, or rehabilitation setting strongly preferred
• Familiarity with discharge planning, psychosocial assessments, and resident care planning
• Strong interpersonal, communication, and organizational skills
• Ability to work independently and collaborate effectively with a multidisciplinary team
• Experience with PointClickCare (PCC) is a plus
