Job Summary
Registered Nurse, Home Healthcare. Provides skilled in-home nursing care to patients through comprehensive assessments, individualized care plans, education, documentation, and coordination with other care disciplines. The role manages an assigned patient caseload while supporting patients, families, and care team members in achieving care goals.
Job Details
- Required Qualifications: Registered Nurse (RN) licensure in the state of practice; Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS), HS-BLS, or RQIBLS certification from an approved vendor; valid driver’s license; at least 21 years of age; current automobile insurance; and access to a reliable automobile.
- Preferred Qualifications: Bachelor of Science in Nursing (BSN) degree; acute care facility experience; or home healthcare experience.
- Schedule: Day shifts, 12-hour shifts, minimum 36 hours per week.
- Additional Information: Provides direct and indirect skilled nursing care in patients’ homes; completes initial and ongoing comprehensive assessments; implements and updates individualized plans of care; coordinates care with other disciplines; documents patient status, care delivery, responses to treatment, and therapeutic interventions; educates patients and caregivers; initiates preventative and rehabilitative nursing procedures; and performs assigned on-call duties based on operational needs.
