Registered Nurse, Case Management Experience. The position supports adult patients through case management and navigation services, including coordination of follow-up care and referrals. Job Details

• Schedule: Day shift, 8 hours per shift, 40 hours per week.
• Required Qualifications: Basic Life Support (BLS) certification; California Registered Nurse license, with pending licenses not accepted; minimum of 2 years of experience; experience with case management and patient navigation.
• Additional Information: Responsibilities may include chart preparation, disability form completion, follow-up phone calls, insurance verification, medication refill coordination, pain assessment, patient history and allergy review, patient teaching, pharmacy calls, referrals, and appointment scheduling. The role supports adult patients, including those diagnosed with sickle cell disease. Blue scrubs are required.
