Job Summary

Registered Nurse, Clinic, Grand Junction, Colorado: The Clinic Registered Nurse provides evidence-based nursing services to participants in the Program of All-inclusive Care for the Elderly (PACE). Responsibilities include physical assessments, care plan development, patient education, counseling, skilled interventions, and coordination of care documentation.

Job Details

• Required Qualifications: Current Colorado Registered Nurse licensure maintained throughout employment; graduation from an accredited school of nursing; minimum of 2 years of Registered Nurse experience; excellent interpersonal, verbal, and written communication skills; ability to manage multiple priorities and maintain focus; proficiency with computers and technology; successful completion of competency evaluations.
• Preferred Qualifications: Bachelor of Science in Nursing preferred; clinic experience preferred.
• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Additional Information: Provides support to participants and their caregivers or family members through teaching, counseling, physical assessments, care planning, and other skilled nursing interventions. Coordinates scheduling, communication, and encounter documentation while complying with applicable regulatory requirements. The role requires effective collaboration, respectful communication during highly emotional interactions, and the ability to interpret written, oral, diagram, and schedule-based instructions.
