Job Summary
Registered Nurse, Case Management, Lynchburg, Virginia. The Care Transitions Registered Nurse is responsible for assessing clinical needs and coordinating safe, appropriate discharge plans for patients in an acute care setting. The role involves collaboration with interdisciplinary teams while supporting individual patient needs and compliance with state and federal regulations.
Job Details
- Schedule: Day shift, 7:30 a.m.–4:00 p.m., Monday through Friday; 8-hour shifts and 40 hours per week. Weekend and holiday coverage may be required.
- Required Qualifications:
- Active Virginia registered nurse license or current multistate licensure privilege
- At least 2 years of experience in a similar care transitions role
- 3 years of experience in acute care nursing
- COVID-19 and influenza vaccination or declination
- Strong computer skills
- Excellent communication skills
- Ability to collaborate effectively with interdisciplinary teams
- Preferred Qualifications: Bachelor of Science in Nursing
- Float: Float rotation may be required.
- Additional Information:
- Primary work location is Lynchburg, with potential coverage in Bedford or Southside.
- Responsible for determining and facilitating appropriate discharge plans.
- Must support patient-specific needs and applicable state and federal regulations.
- Start date: 11/30/2026.
- Assignment end date: 02/27/2027.
