Pay as Cross Country posts it: about $2,513 a week in all, a taxable rate of $21.88 an hour plus $1,638 a week in housing and meal stipends.

Job Summary

Case Management Exp provides comprehensive care coordination for pediatric patients across the health care continuum. The Case Manager assesses care needs, develops and monitors individualized plans, coordinates interdisciplinary services, evaluates medical necessity, and facilitates patient discharge planning. The position also identifies barriers to effective care and supports quality clinical outcomes.

Job Details

• Required Qualifications: Bachelor’s degree in case management or health care; current Registered Nurse (R.N.) license in the state worked; Basic Life Support (BLS) certification for acute care assignments; 3–5 years of clinical experience with working knowledge of care management, acute care or home care environments, community resources, and resource utilization management; minimum of one year of experience in an acute care hospital or home care setting for registry and travel assignments; recent case management experience in an acute care or home care setting within the past 12 months; recent pediatric acute care experience; more than two years of recent adult health case management experience; demonstrated critical thinking, problem-solving, communication, time management, and interdisciplinary collaboration skills.
• Preferred Qualifications: Certified Case Manager (CCM) certification; additional related education or experience.
• Schedule: Day shift, 10-hour shifts, with a minimum of 40 hours per week; rotating weekend coverage may be required.
• Additional Information: Provides pediatric case management in a Level I trauma care environment. Responsibilities include coordinating patient services, developing and communicating interdisciplinary discharge plans, evaluating the appropriateness of care, identifying and resolving discharge barriers, collaborating with physicians, payers, community agencies, and regulatory agencies, and educating health care team members on case management concepts. The position may supervise other staff and requires the ability to meet direct patient care physical demands, including frequent standing, walking, bending, reaching, and assisting patients with mobility.
