Job Summary

Case Manager, Case Management Experience, Rochelle Park, NJ. This role coordinates patient care, identifies barriers to treatment and discharge, and collaborates with patients, families, physicians, and interdisciplinary team members to support safe, timely transitions to the next appropriate level of care. The position also provides case management, utilization review, discharge planning, resource coordination, and clinical documentation.

Job Details

• Schedule: Day shift, 8-hour shifts from 8:00 a.m. to 4:30 p.m., with a minimum of 40 hours per week. The role requires work the day after Thanksgiving, Christmas Eve, and New Year’s Eve.
• Required Qualifications:
• Current licensure in a clinical discipline in accordance with state guidelines, such as registered nurse, respiratory therapist, or social worker.
• Working knowledge of the insurance industry and government reimbursement processes.
• Two years of experience in case management and discharge planning in an acute care setting.
• Additional Information:
• Identify patient and family needs that affect participation in the treatment plan.
• Develop safe discharge plans in collaboration with patients, families, physicians, and the interdisciplinary team.
• Coordinate referrals to community resources and outside agencies when appropriate.
• Complete utilization review, discharge planning evaluations, care plans, progress notes, and other required clinical documentation.
• Participate in team conferences, scheduled meetings, data collection, and quality-improvement activities.
• Promote patient and employee safety by following established safety procedures and identifying potential hazards.
• Interact appropriately with patients and families according to age and level of understanding.
• Submit accurate and timely documentation and reports in accordance with applicable policies and regulations.
