Job Details

• Required Qualifications: Minimum 5 years of hospital case management experience and 5 years of hospital acute adult care nursing experience; demonstrated experience in discharge planning and care coordination.
• Preferred Qualifications: More than 5 years of hospital nursing practice.
• Schedule: Day shift, 8 hours per shift, 40 hours per week; Monday through Friday, with every third weekend and holiday work required.
• Float: Float rotation may be required.
• Additional Information: The role supports a high-acuity, complex patient population with a high percentage of behavioral health patients. Business casual or professional casual attire is acceptable.
