Job Summary

Case Management Exp, Case Management - This position supports acute care case management and discharge planning for neonatal, adolescent, adult, and geriatric patient populations. The role involves care coordination, utilization review, treatment planning, patient assessments, and collaboration with interdisciplinary teams in an acute care setting.

Job Details

• Schedule: Day shift, 8-hour shifts from 8:00 a.m. to 4:30 p.m., 40 hours per week, with weekend rotation requirements, including every other weekend and approximately 4–5 weekend shifts within a 6-week period.
• Required Qualifications:
• At least 2 years of acute care case management or discharge planning experience
• Hospital experience required; behavioral health experience alone does not meet the requirement
• Travel experience required; seasoned travelers only
• Valid California state license required by the start date; pending licensure may be accepted before the start date
• Experience with acute hospital care, admission criteria, benefits eligibility, care coordination, discharge planning, disease management, emergency department, intensive care unit, medical-surgical services, needs assessments, durable medical equipment coordination, plan of care development, prior authorizations, and surgical services
• Knowledge of Centers for Medicare and Medicaid Services (CMS), diagnosis-related groups (DRG), The Joint Commission standards, core measures, and national safety goals
• Ability to independently assess patients, conduct utilization review, coordinate care, arrange durable medical equipment, skilled nursing facility placement, and ambulance transportation
• Competence in treatment planning, family meetings, and coordination for patients with complex social and medical needs
• Additional Information:
• Caseload of up to 25 patients
• Patient population includes neonates, adolescents, adults, and geriatrics
• Collaboration with physical therapy and social services
• Knowledge of managed care organizations, independent practice associations, adoption and foster care resources, substance use treatment options, homelessness-related services, mental health crises, advance directives, Physician Orders for Life-Sustaining Treatment, and protective services processes
• Forty hours of orientation
• Business attire or modified scrubs
