As an LPN Licensed Practical Nurse, you will play a vital role in delivering exceptional patient care to individuals in our Scranton, PA facility.
Job Summary
We are seeking a compassionate and skilled LPN Licensed Practical Nurse to join our dedicated healthcare team. As an LPN Licensed Practical Nurse at Carbondale Rehabilitation and Healthcare Center, you will provide high-quality patient care, support, and education to our residents, while working closely with our interdisciplinary team to achieve optimal outcomes.
Responsibilities
- Provide direct patient care, including medication administration, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Assist with activities of daily living, such as bathing, dressing, and grooming
- Monitor and report any changes in patient condition to the nursing team
- Maintain accurate and timely records of patient care and treatment
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Collaborate with other healthcare professionals to ensure seamless care transitions
Requirements
- Current LPN licensure in the state of Pennsylvania
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment with frequent interruptions
- Familiarity with electronic medical records systems (EMRs)
