About the Role
As a Care Manager, you will be responsible for coordinating the delivery of home care services to our clients. You will work closely with our clinical team to create and implement personalized care plans, ensuring that our clients receive the highest level of care in the comfort of their own homes. Your primary focus will be on assessing the needs of our clients, identifying potential problems, and developing strategies to address them. Your exceptional communication and interpersonal skills will be essential in building strong relationships with our clients, their families, and our care staff.
Responsibilities
- Conduct initial and ongoing assessments of clients to identify their care needs and develop personalized care plans
- Collaborate with the clinical team to create and implement care plans that meet the unique needs of each client
- Coordinate with care staff to ensure that services are delivered according to plan and that any issues are promptly addressed
- Identify potential problems and develop strategies to address them, working closely with the clinical team to ensure that clients receive timely and effective care
- Communicate effectively with clients, their families, and care staff to ensure that everyone is informed and engaged in the care planning process
- Monitor client progress and adjust care plans as needed to ensure optimal health and wellness
