About the Role

As an LVN on our team, you will be responsible for providing direct patient care, administering medications, and performing various clinical tasks. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our patients receive the highest level of care. Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and conducting wound care
• Develop and implement individualized care plans in collaboration with our interdisciplinary team
• Conduct patient assessments, identify care needs, and communicate with the healthcare team to develop effective care strategies
• Administer oxygen therapy, perform EKGs, and conduct other clinical procedures as needed
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives to ensure our high standards of care
