Salary: $85K to $95K range (based on experience)
Responsibilities of the Intake RN - CHHA
- Review incoming clinical records and patient histories to evaluate medical necessity and clinical appropriateness for home health services
- Process initial physician orders and verify mandatory documentation according to state and federal regulations
- Build preliminary care plans and assign visiting disciplines based on patient acuity and needs
- Act as the primary liaison between hospitals, discharge planners, physicians, internal clinical teams, and the patient's family
- Explain what to expect from home care services, answer initial questions, and provide guidance on patient rights and agency processes
Qualifications of the Intake RN - CHHA
- Must have current New York State RN license
- Must have at least two years of experience in a CHHA intake department or MLTC intake all scripts experience
- Must have a minimum of one year supervisory experience
- Must have excellent clinical, writing and communication skills
