About the Role

As a Physical Therapist (PT) at York Nursing and Rehab Center, you will have the opportunity to make a positive impact on the lives of our patients. As a key member of our rehabilitation team, you will be responsible for providing high-quality physical therapy services to patients with varying levels of physical disability. Responsibilities

• Design and implement individualized physical therapy treatment plans to meet the unique needs of each patient
• Conduct patient evaluations and assessments to identify areas of strength, weakness, and dysfunction
• Develop and implement therapeutic exercises, stretches, and activities to improve mobility, strength, and function
• Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal patient outcomes
• Document patient progress and treatment plans accurately and timely
• Participate in patient education and discharge planning to ensure smooth transitions to the next level of care

Requirements

• Bachelor's or Master's degree in Physical Therapy
• Current licensure as a Physical Therapist in the state of Pennsylvania
